The only way to get a cash Settlement Payment from the Settlement is to submit a Claim Form.
Claim Forms must be filed online or mailed and postmarked on or before December 31, 2023, to: Gulf Coast Injury Center v. Esurance Claims Administrator, P.O. Box 2480, Portland, OR 97208-2480.

Exclude Yourself from the Settlement by October 15, 2023.

Get no Settlement benefits. Keep your right to file your own lawsuit against the Defendants about the legal claims in this case. To opt out of the Settlement, you must submit a written opt-out request, which must be mailed and postmarked by October 15, 2023. You cannot opt out by telephone or by email.

Object to the Settlement by October 30, 2023.

Tell the Court why you do not like the Settlement. You will still be bound by the Settlement if the Court approves it. You must file your objection with the Court by October 30, 2023, with copies to Class Counsel and Defense Counsel.

Attend a Hearing.

Ask to speak in Court about the fairness of the Settlement. At this hearing, the Court will consider whether the Settlement is fair, reasonable, and adequate to the Settlement Class.
If you send an objection, you do not have to come to Court to talk about it. As long as you mailed/submitted your written objection on time, the Court will consider it.
If you intend to speak at the Final Approval Hearing, you must follow the procedures set forth in Faq 19 and Faq 20 when you serve your objection.

Do Nothing.

Get no cash Settlement Payment from the Settlement. Be bound by the Settlement.
